  <abstract>This study aims to determine the learners' academic performance in English and difficulties encountered in solving Mathematical word problems upon transitioning in the use of the medium of instruction. It utilized a convergent mixed method research design using a researcher-made test and in-depth interview for the Grade IV learners of selected public schools in Tagaytay City. There were 167 learners for quantitative data and 10 for qualitative data. The result shows that most of the respondents got a grade of below 75, meaning most did not meet expectations in solving word problems in Mathematics using English as a medium of instruction. Some difficulties exist among respondents upon transitioning the medium of instruction from Filipino to English in solving word problems in Mathematics. This means that most of the learner respondents did not perform well in solving Mathematical word problems expressed in the English language. Thus, the proposed transition plan is recommended for all Grade IV learners.</abstract>
