This study intended to assess the effectiveness of the Guidance Program in Colegio de Sta. Rosa - Makati with an end of crafting an intended Guidance Manual for program development and sustainability. This study utilized quantitative descriptive evaluative method using a questionnaire among two hundred (200) respondents consists of ninety (90) students; ninety (90) parents; and twenty (20) teachers. Data were treated using statistical formulae for frequency, percentage, rank, weighted mean and one-way ANOVA for this purpose. Findings revealed that the Guidance Program of Colegio Sta. Rosa - Makati was perceived to be moderately effective along individual information, orientation, counseling, testing, homeroom guidance, career guidance, research and evaluation, and academic guidance. As such, there was no established great difference among respondents' perceptions on such program effectiveness. Hence, rapport with guidance facilitator, lack of interactive homeroom activity, time constraint, academic concern, and unclear information dissemination were identified to impede guidance service operation. Thus, an intended manual that contains provisions, policies, functions, responsibilities and services toward program development sustainability was crafted to address such identified concerns.
